Act Respecting the Protection of Personal Information in the Private Sector (LPRPSP)
The employees and therapists at the Santé Globale Châteauguay clinic are committed to respecting their
clients’ privacy and complying with the new obligations and directives surrounding Bill 25. Santé Globale
Châteauguay has implemented a strict procedure for the retention and destruction of personal information to
ensure the protection of its clients’ privacy and prevent confidentiality-related incidents. Our priority is to
maintain our clients’ trust and protect the clinic’s reputation.

Privacy Policy
When you provide personal information, whether when booking an appointment or during a session with one of
the clinic's professionals, you consent to its processing and use by other members of the Santé Globale
Châteauguay team (assistant, therapist). The personal information we collect about you is used to create your
file and ensure optimal follow-up. Once you have given your consent, you can withdraw it at any time. This
withdrawal may apply to only part of the information collected, or to all of it.
All these measures are in addition to those already in place within the framework of the codes of ethics and
professional conduct to which all our therapists must adhere.
Personal Information Retention and Destruction Procedure
To ensure the protection of our clients' privacy and comply with legal obligations regarding the protection of
personal information, we keep our clients' files in a locked filing cabinet located behind the reception desk.
Only the administrative assistant and the clinic's therapists have access to it. Client files will be destroyed
when they have been inactive for at least 7 years.
Procedure for requesting access to your file
You can always request a copy of your file. To do so, you must submit a written request to the Privacy Officer.
The request can be sent by email or postal mail. Once the request is received, it will be analyzed, and the
client's identity will be verified. Once the verification is complete, the requested copy of the file will be sent to
you by email, secure postal mail, or delivered in person.
Procedure in the event of a complaint or data breach
Do you believe your privacy has been breached? Do not hesitate to contact the Privacy Officer in person, by
phone, or by email. A complaint form will be completed and recorded in our registry. The complaint will be
analyzed, and solutions will be proposed as soon as possible. All information related to the complaint will be
kept in a confidential file.
In the event of a breach of personal information, we undertake to document the incident and enter it in the
confidentiality incident register. If we determine that there is a risk of serious harm to the individuals concerned, we will also report the incident to the Commission de l'accès à l'information du Québec.
Appointment Cancellation or Rescheduling Policy
In order to provide quality service to our clients and out of respect for our therapists, if you need to cancel or
reschedule your appointment. Santé Globale Châteauguay asks that you notify reception at 450-692-4881 as
soon as possible, and at least 24 hours before your scheduled appointment.
IMPORTANT: If an appointment is canceled or changed less than 24 hours in advance, you will be charged a
fee ranging from $80 to $150 (depending on the service booked).
Civil and Professional Liability
The Santé Globale Châteauguay clinic requires each of its therapists to hold civil and professional liability
insurance.
